HOW DO I APPLY FOR A PASSPORT
The Town Clerk’s office has been designated as a passport agency
You must appear with a long form birth certificate that contains a date of record
and a raised seal, and a valid driver’s license.
Two 2 x 2 photos are required. Photos can be taken in the Town Clerk’s Office.

FEES: Application Fee (Age 16 & Over) $110.00
Application Fee (Age 15 and Under) $ 80.00
Execution Fee (retained by the Town) $25.00
Photograph Fee (retained by the Town) $10.00
PASSPORT CARD FEE: (for land/sea travel only)
Application Fee (Age 16 & Over) $30.00
Application Fee (Age 15 & Under) $15.00
Payment for Passport Book/Card has to be in the form of a
Check or Money Order
Execution fee and photograph fee retained by Town
can be paid by cash or check (made payable to Town of
East Bridgewater). Total fee $35.00 (incl. pictures)
